About This Program
Premise:
To ensure comprehensive cancer care meeting the psychosocial needs of patients and their families both individually and on a systemic level.
Vision:
To optimize patient and family care using a comprehensive, compassionate and coordinated care model to meet their psychosocial needs.
Referral Process
Initiated by health care team, patient, or family and communicated to social work team via Meditech referral, phone, or email.

Referral recommended for patients scoring at a 7 or above on the pain, depression, anxiety, or well-being on the Edmonton Symptom Assessment System.
Team will triage referrals with goal of making contact within 1-3 days based on acuity and urgency. For patients where the need is urgent please contact social work Professional Practice Lead. If the patient is in the clinic and you would like them seen today, please call as soon as possible for the social work team to attempt to meet this need.
Social work notes are documented in PCI in Patient Care Notes.
Resources
Social workers provide services to Medical Day Clinic clients directly and also partner with and refer to community supports. Information on common resources are being made available on various pages under the Cancer Care page and on pocket cards available in the clinic.
